If you are a member of either of these insurance programs, you need to provide your information to Jackie Johnson, Membership Coordinator, to assure that your Center membership is up-to-date. ALL SILVER SNEAKERS must let Jackie know if your insurance will continue to provide Silver Sneakers. If you have not yet done so, please contact Jackie with this information: Your Name, Date of Birth, which program your insurance provides (Silver Sneaker or Silver & Fit), the type of insurance you have (eg., UCare).
